$70 HEAD COACH REFUND
To receive your $70 refund check at the February 8, 2020 coaches meeting all of the following must be done:
- Complete Risk Management approval
- Obtain appropriate Grassroots license level (4v4, 7v7, 9v9, 11v11)
- U5-U8 coaches must complete the minimum of grass roots introduction (unless previously taken) plus 4v4 online class; class is approximately 2 hours
- U10 coaches must complete the minimum of grass roots introduction (unless previously taken) plus 7v7 online class; class is approximately 2 hours
- U12 coaches must complete the minimum of grass roots introduction (unless previously taken) plus 9v9 online class; class is approximately 2 hours
- U14-U18 coaches must complete the minimum of grass roots introduction (unless previously taken) plus 11v11 online class; class is approximately 2 hours

Risk Management Procedure
Definition Team Administrator: ANY and ALL persons engaging in an administrative capacity within the YOUTH sports program. Including but not limited to Board Member, Team Parent, Head Coach, Assistant Coach and Referee.
1.) Fill out the questionnaire below and your information will be sent to the MYSL Registrar after you submit it.
2.) Send a picture of your driver license to firstname.lastname@example.org
3.) Set-up an appointment with UPS OR Murrieta Police Dept. (951)304-2677. Costs vary, should be around $20, so please ask the provider what they charge and type of payment they accept. The UPS store listed below charges $15 for Murrieta Youth Soccer League livescans. Let them know you were referred there by the league registrar.

4.) Please Complete, Print & Sign 2 copies of the following form & take with you to your Livescan appointment.
Complete the form, enter the following information pertaining to our league:
League Name: Murrieta Youth Soccer League, Team Name: leave it blank.
5.) Email or fax the livescan form after you’ve had your finger prints processed.
6.) The following forms must also be completed, printed, signed and submitted via fax or email ASAP to the registrar to save your spot in line.
As You complete the form, enter the following information pertaining to our league:
SEASON: Will either be e.g. Spring 2017 or Fall 2017-2018
TEAM ID Number: leave it blank (team number are assigned at a later time & change each season)
MANDATORY ONLINE CONCUSSION CLASS:
Required Concussion Class Information
All Head Coaches, Assistant Coaches, Team Managers & Team assistants must complete the Heads Up to Youth Sports Concussion Training Course & upload to their Calsouth account their Concussion Certificate, which will be available at the end of the course.
HEADS UP Concussion in Youth Sports is a free, online course available to coaches, parents, and others helping to keep athletes safe from concussion. It features interviews with leading experts, dynamic graphics and interactive exercises, and compelling storytelling to help you recognize a concussion and know how to respond if you think that your athlete might have a concussion.
Once you complete the training and quiz, you’ll be able to save and upload the certificate to your Calsouth file (for instructions, see video below).
If you have already completed the course & uploaded the concussion certificate thank you. You don’t need to do anything else. Below you’ll find the links for 2 sites where you can complete the course & instructions on how to upload the concussion certificate to your Calsouth.com account.
- CDC Heads Up Concussion:
CDC Heads Up Concussion module is an online course with a duration of about 30 minutes in length. Your admins will create a profile account with their system and be sent an email confirmation to be able to start the course. Once complete, they will be able to save and print their certificate. - National Federation of High Schools (NFHS):
National Federation of High Schools (NFHS) has partnered with the Center of Disease Control (CDC) to offer Concussion in Sports courses. Your admins can create a profile within that website and will quickly be able to order a course. Their certificate will always remain within that account and can be printed as well. An added feature to this website is the ability to order this free course for others to take. At that point, you can then monitor their progress and view when they have completed it. SAFESPORT TRAINING & EDUCATION
It’s mandatory for all team administrators (head & assistant coaches, team managers and team assistants) to take the U.S. Center for SafeSport training and education online course. The course takes about 90 minutes to complete.
The correct certificate will read: For the Successful completion of SafeSport Trained.
Upload to your Calsouth file the certificate you receive at the end of the class. The SafeSport Trained certificate is uploaded by using the same process as uploading the Concussion Education Certificate. Below is a YouTube tutorial video link.

The certificate is required before a team can be assigned to you.
Do not wait until the last minute to take the class, take it now to avoid the rush (and slow to load problems) that often occur when everyone waits until the last minute to complete.
Before submitting your paperwork please review to ensure you’ve dated & signed the forms.

Important Note: Fax /email the Livescan form only after you’ve been finger printed. The rest of the forms are sent now. Please note that Calsouth will not accept livescans processed for other organizations. Livescans processed for Calsouth are good for life.
Contact the Director of Recreational at, email@example.com or Director of Coaches at MyslCoaching@gmail.com to obtain the code needed to take the free online 4v4 license course. State in email that you’ve already turned in your paperwork to the registrar. Full or partial compensation of the fee paid for the 9v9 and 11v11 license course may also be available, please ask if interested.
